Long Form or Short Form... which one's right for me?

When we originally created the checklist in 2010, we started by breaking down everything we mentally asked or thought about while doing Level 1 and Level 2, plus what is required by NFPA 211. We ended up with a 4-to-6-page checklist, which sounds daunting, but it's a written-out version of what goes through your head. This is what we call the Long-form; it's now 4 legal-sized pages with room for notes. 

We then took that form and rehashed every question to create the Short Form with is 1-page front and back or 2 pages.
The short form has all the required questions and additional questions that help you gather data.

We offer both versions as every person, and every company is different. 
There are 4 - Long forms and 8-Short Forms. 

Long-form categories:

  • Masonry Fireplace
  • Prefab Fireplace
  • Masonry Thimbled Chimney Serving an Appliance
  • Prefab Chimney Serving an Appliance
Short forms have 6 sub-category forms from the original 4; these are:
  • Masonry Fireplace with Hearth Appliance
  • Masonry Fireplace with an Insert
  • Masonry Thimbled Chimney serving an Appliance - Liquid Fuel
  • Masonry Thimbled Chimney serving an Appliance - Solid Fuel
  • Prefab Chimney Serving an Appliance - Liquid Fuel
  • Prefab Chimney Serving an Appliance - Solid Fuel
These extra categories make it easier to go through the forms without unnecessary questions. Example - on a solid fuel form, you wouldn't have a question about gas lines.
