Our Standard Do-Confirm Form​ for Chimney and Fireplace Inspections - Levels 1 and 2

A Our forms are segmented by "Green Boxes). These boxes include the Urban Inspection Language™ Code to easily look up and insert language paragraphs that apply when creating a report.
Our Flue & Hearth Notes™ Standard Do-Confirm chimney inspection forms include eight categories. The categories for these forms are:
  • Masonry Fireplace
  • Masonry Fireplace with Hearth Appliance
  • Masonry Fireplace with an Insert
  • Masonry Thimble Chimney serving an Appliance - Liquid Fuel
  • Masonry Thimble Chimney serving an Appliance - Solid Fuel
  • Prefabricated Chimney Serving an Appliance - Liquid Fuel
  • Prefabricated Chimney Serving an Appliance - Solid Fuel
  • Prefabricated Chimney & Fireplace
​These extra categories make it easier to complete the forms without unnecessary questions. Example - on a solid fuel form, you wouldn't have a question about barometric dampers.

Within these forms, the inspection items are broken down into subcategories called green boxes. These boxes contain areas and items to be inspected and also include a code to cross reference to our Urban Inspection Language™. We have an example of a green box shown above right.
At the bottom of the page, we have some samples of our inspection forms to give you an idea of what they look like. Our forms cover both Levels 1 and 2.

A fireplace with an insert appliance connects from the appliance and exits the flue.
A hearth stove is connected to a chimney liner that runs to the top of the chimney.
Sometimes, Summary Report Forms are used in conjunction with these Do-Confirm Forms (above) to summarize the condition of the chimney and/or fireplace. Then the technician files the Do-Confirm for future reference.
